Error while login into MDM Web page

Dear Team,
We are trying to login into MDM Web page http://localhost:8080/org.eclipse.mdm.nucleus using sa/sa.

We are unable to login and gives the message Invalid User Id or Password.

Server log file gives the error message as :
java.lang.IllegalStateException: Duplicate key GlobalFlag

We are using Avalon ASAM-ODS server 4.7.
It is connected to the Oracle 12c DB, existing application model for OpenMDM4.

Please find the attachment for logs generated at
glassfish-4.1.2 (1)\glassfish4\glassfish\domains\domain1\logs\server.log

Please suggest to resolve this issue.

Going forward we will be migrating from OpenMDM4.0 to OpenMDM5.
Please let us know if there are any recommendations required for migrating OpenMDM4.0 to OpenMDM5
